The Support Coordinator is a varied role that will oversee NDIS Support Coordination in Metro South East region. It is responsible for ensuring relevant support services are delivered consistent with organisation's policy and procedures, organisational values, the Victorian Disability Service Standards and relevant Occupational Health and Safety requirements.
The role will:
- provide assistance to strengthen customer abilities to participate more fully in the community
- coordinating a range of supports
- resolving customer issues
- initial assistance with linking participants with the right providers
- assistance to source providers
- developing participant resilience in their own network and community
We are looking for an experienced professional with a minimum Cert IV in Disability and experience in a support coordination role.
